Key Responsibilities
  • Engage creatively in therapeutic interventions tailored to the needs of clients, utilizing evidence-based techniques such as Cognitive Behavioral Therapy (CBT), Acceptance and Commitment Therapy (ACT), and Motivational Interviewing (MI).
  • Provide comprehensive documentation for each client session in accordance with company guidelines; ensure all records reflect accurate therapeutic progress, including treatment plans and case notes.
  • Conduct a minimum of 25 client-facing visits weekly to ensure patients receive quality care, fostering a balance between professional responsibilities and personal well-being.
  • Work collaboratively with case managers, families, and interdisciplinary teams including medical personnel to coordinate comprehensive care services for clients.
  • Participate fully in clinical staff meetings and training sessions to enhance your professional skills and contribute to ongoing service improvement.
  • Engage in community outreach initiatives to establish relationships that strengthen service delivery and support for those in need.
  • Respond promptly to inquiries and communications from the Clinical Director, establishing efficient lines of communication.
  • Actively support team dynamics while valuing individual autonomy within your clinical practice.
Patient Population / Client Focus

The focus of this role is primarily on individuals and families receiving hospice care, including those facing life-limiting diagnoses. You will work closely with clients from diverse backgrounds and identities, providing culturally competent care that respects their unique experiences. Qualifications
  • A Masterโ€™s degree in Social Work, Counseling, Psychology, or a related mental health discipline is required.
  • Must possess an independent license as a Licensed Clinical Social Worker (LCSW).
