• Conduct thorough mental health assessments of patients and their families, evaluating emotional and psychological needs in the context of advanced illness.
• Design and implement individualized treatment plans that leverage evidence-based therapeutic interventions to enhance emotional wellbeing and coping mechanisms.
• Utilize therapeutic modalities such as cognitive-behavioral therapy (CBT), mindfulness, and acceptance-based strategies tailored to the unique challenges faced by hospice patients.
• Facilitate online support groups and individual sessions that encourage open dialogue about grief, loss, and anxiety, helping families navigate their experiences.
• Collaborate with a diverse team of healthcare professionals, including nurses, physicians, and spiritual counselors, to ensure comprehensive and holistic patient care.
• Provide crisis intervention as necessary and develop proactive strategies to manage acute emotional distress among patients and families.
• Maintain accurate and detailed documentation of assessments, treatment plans, progress notes, and any intervention outcomes in compliance with hospice regulations and standards.

In this role, you will primarily work with patients who are facing terminal illnesses and their families. You will provide support tailored to individuals from diverse backgrounds, focused on emotional resilience, grief support, and facilitating quality end-of-life care. Your contributions will be vital in helping families understand the dying process, cope with loss, and emerge with a sense of closure and peace.

• A Bachelor's Degree in Social Work from an accredited institution.
• Active Licensed Clinical Social Worker (LCSW) certification.
• Strong proficiency in mental health assessments and evidence-based therapeutic practices.
• Excellent communication and interpersonal skills to foster relationships with patients and their families.

• Prior experience in hospice or palliative care settings, demonstrating an understanding of the complexities involved in end-of-life issues.
• Experience working with diverse populations and addressing culturally specific needs during critical life transitions.
• Familiarity with telehealth platforms and remote therapy modalities.

The salary range for this position is between USD 80,000 and 100,000 per year, commensurate with experience and qualifications. Additionally, we offer a comprehensive benefits package that includes:
