This role is designed to be fully remote, providing you the flexibility to manage your work-life balance while delivering critical social work services. The position supports clients in hospice settings, requiring a compassionate understanding of end-of-life issues and mental health concerns. You will work collaboratively with healthcare providers, families, and community resources to develop and implement effective care strategies.

• Directly engage with clients experiencing major mental illnesses and severe emotional disorders through assessment and therapy sessions conducted primarily via telehealth.
• Develop and implement integrated care plans in collaboration with clients, family members, and interdisciplinary teams, ensuring that all aspects of a client’s needs are met.
• Provide a variety of brief, focused services, including individual, group, and family therapy tailored to the needs of clients facing end-of-life challenges.
• Conduct thorough and developmentally appropriate assessments to formulate effective treatment strategies while adhering to ethical guidelines.
• Assist in crisis intervention efforts and provide timely support and guidance during critical moments.
• Monitor client progress, ensuring equitable access to services, timely follow-ups, and completion of necessary reporting and data submissions.
• Collaborate with healthcare teams to create sanctions to ensure safe environments for clients throughout their hospice experience.
• Engage in continuous professional development to enhance skills and improve outcomes for clients.

The Licensed Clinical Social Worker will primarily serve individuals in hospice care, supporting clients and their families as they navigate complex emotional and psychological issues associated with terminal illness. You will work with diverse populations, respecting different traditions, beliefs, and interpersonal styles. As you enhance the lives of your clients, you will play a crucial role in facilitating empowerment and choice, maintaining their dignity and self-determination throughout their care journey.

• A Bachelor’s Degree in Social Work (BSW) from an accredited institution is required.
• Current licensure as a Clinical Social Worker (LCSW) in the state of Pennsylvania is essential.
• A minimum of 2 years of related experience in a clinical setting, preferably working with hospice or palliative care clients.

• Prior experience in a hospice care environment or with clients facing major mental health challenges is highly desirable.
• Experience within interdisciplinary teams focusing on mental health and social services will be an advantage.
• Familiarity with telehealth practices and remote service delivery.

The salary range for this position is between USD 70,000 and 85,000 annually, commensurate with experience and qualifications.
