• Conduct thorough assessments of clients’ social, emotional, and psychological wellbeing to inform tailored support plans.
• Provide individual and group counseling sessions using evidence-based therapeutic approaches, ensuring a safe space for expression and healing.
• Develop and execute personalized service plans aiming to meet clients' specific goals, supporting their journey toward improved mental health and social stability.
• Advocate vigorously for clients' rights, ensuring access to necessary resources while addressing broader social justice issues that affect client well-being.

The primary focus of this role includes individuals and families navigating various life challenges, particularly those related to end-of-life care and emotional distress commonly associated with hospice services. As an LCSW, you will be at the forefront of providing compassionate support and counseling, helping clients cope with grief, loss, and transitions inherent in the hospice experience.

• A Master's degree in Social Work from an accredited institution is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Strong documentation and assessment skills to accurately build and track client cases.
• Knowledge and understanding of trauma-informed care principles and social justice frameworks are crucial.
• Exemplary communication, empathy, and organizational skills, with a keen ability to navigate sensitive conversations.
