• Conduct thorough psychosocial assessments to identify clients' mental health concerns, strengths, and needs, employing best practices tailored for hospice care.
• Provide individual therapy and group sessions using evidence-based treatment modalities such as cognitive-behavioral therapy, trauma-focused therapy, and solution-focused therapy.
• Collaborate closely with psychiatrists, psychologists, and medical teams to formulate and implement comprehensive, individualized treatment plans that address unique challenges faced by hospice patients.
• Facilitate therapeutic interventions that help clients and their families navigate the emotional complexities of end-of-life decisions and grief.
• Utilize crisis intervention techniques—conducting risk assessments and safety planning to manage potential issues that may arise in vulnerable patients.
• Deliver psychoeducation to clients and families, empowering them with coping strategies, grief support, and wellness enhancement techniques.
• Maintain accurate and timely clinical documentation as per ethical and legal standards, ensuring all information is kept confidential and aligns with best practices in social work.
• Contribute to the development and facilitation of support groups catered specifically to individuals dealing with loss, bereavement, and transition.

• A Master’s degree in Social Work from an accredited institution.
• Current active state licensure as a Licensed Independent Clinical Social Worker (LICSW).
• Exceptional interpersonal, communication, and assessment skills, with a strong aptitude for empathetic engagement.
• Proven knowledge of evidence-based therapeutic modalities relevant to hospice care.

• A minimum of two years of clinical experience in a mental health or outpatient setting, with a strong preference for experience in hospice or palliative care.
• Experience in developing, implementing, and evaluating treatment plans in collaboration with multidisciplinary teams.
• Background in crisis intervention and experience working with diverse populations in sensitive situations.
