Sonic Multi Motion Rehab and Wellness Center is seeking a dedicated and licensed mental health counselor to join our team as a full-time Integrated Care Specialist. This role is structured as a remote position, making it accessible and convenient while maintaining a direct connection to our community health clinic. • Conduct detailed mental health assessments that take into account patients’ medical conditions impacting their mental health.
• Develop and execute personalized treatment plans that address both psychiatric needs and co-occurring medical conditions.
• Facilitate individual and group therapy sessions, employing evidence-based therapeutic techniques tailored to each patient’s unique circumstances.
• Collaborate closely with medical providers to create a unified treatment approach that optimizes patient outcomes.
• Provide immediate crisis intervention during mental health emergencies, employing de-escalation techniques and supportive strategies.
• Educate patients on the interconnectivity of mental and physical health, empowering them to manage their overall well-being effectively.
• Maintain and manage accurate patient records using electronic health record systems, ensuring compliance with legal and ethical guidelines.
• Engage in team meetings and interdisciplinary care planning discussions, contributing insights and strategies for improved patient care.
• Continuously pursue knowledge on best practices and advancements in integrated care.

We specifically support individuals grappling with co-occurring medical conditions, including autoimmune disorders, chronic pain issues, and substance use disorders. Our ideal candidates should demonstrate a deep understanding and empathy towards patients who may be experiencing the complexities of mental health while managing other health challenges. You will be integral in guiding these patients through their recovery journeys, focusing on empowering them to achieve holistic wellness.

• A master's or doctoral degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.
• Current and valid licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Psychoanalyst, Psychologist, or Licensed Marriage and Family Therapist (LMFT) in New York.
• Strong interpersonal and communication skills, with the ability to build rapport with diverse patient populations.
• Proficient in maintaining professional boundaries while ensuring patient confidentiality.
• Experience with electronic health record systems and standard computer applications to maintain patient documentation and facilitate care.
• Capable of thriving in an interdisciplinary team environment, exhibiting flexibility and cooperation.

Ideal candidates will have previous experience working with individuals who have co-occurring medical conditions. Familiarity with holistic health practices and interdisciplinary treatment approaches is also beneficial. Having continued education or specialized training in trauma-informed care or integrated care methodologies will be considered advantageous.
