Position Overview

Second Chance Health Center is excited to announce an opening for a dedicated and compassionate Licensed Clinical Social Worker (LCSW). In this full-time remote role, you will deliver essential social work services that contribute to our clientsโ€™ mental health and wellbeing. As a part of our team, you will leverage trauma-informed therapeutic techniques to provide telehealth services to individuals navigating challenges related to substance use disorders and trauma.

Work Environment

This full-time position offers the opportunity to work remotely, enabling you to connect with clients from the comfort of your own office while still being part of a dynamic health-focused community. Key Responsibilities
  • Conduct thorough assessments to determine clients' needs and develop individualized treatment plans utilizing evidence-based practices.
  • Provide telehealth mental health services, including counseling and therapeutic support to individuals facing various personal and social challenges.
  • Document and assess medical necessity to justify treatment requirements and support seamless client entry into our programs.
  • Research, refer, and advocate for necessary community resources, helping clients access support such as food assistance, healthcare services, and childcare.
  • Respond to crisis situations effectively, including intervention in cases of child abuse, mental health emergencies, and substance use crises.
  • Utilize tracking systems to monitor progress and follow up on client outcomes, ensuring adjustments are made to treatment plans as necessary.
  • Support clients in adjusting to significant life changes such as illness, divorce, or unemployment through empathetic counseling and guidance.
Patient Population / Client Focus

You will be serving a diverse clientele, which primarily includes individuals grappling with mental health challenges, substance use disorders, and those from historically marginalized groups. Additionally, a portion of our clientele encompasses veterans, requiring specialized understanding and approach towards their unique circumstances. Your role will be pivotal in facilitating access to services that encourage recovery, resilience, and overall wellbeing.

Qualifications
  • Masterโ€™s Degree in Social Work (MSW) from an accredited institution.
  • Valid Licensed Clinical Social Worker (LCSW) licensure in California.
  • Minimum of two years of hands-on experience providing mental health services, particularly within health-related settings.
  • Demonstrated knowledge of various mental health conditions, treatment modalities, and best practices in psychological care.
  • Exceptional listening and interview skills necessary for exploring patient needs and developing tailored treatment strategies.
  • Strong verbal and written communication skills, with the ability to educate and support clients through complex information.
Preferred Experience
  • Experience working with veterans and understanding the unique mental health needs related to this population.
  • Familiarity with community resource networks and advocacy strategies.

Compensation and Benefits
