Key Responsibilities
  • Conduct thorough, ongoing assessments of client needs through telehealth sessions, ensuring personalized treatment plans are created and consistently implemented.
  • Actively communicate progress regarding clients between supervisors, care teams, and family members, facilitating a comprehensive understanding of client development.
  • Develop and present a structured schedule for client appointments, home visits, and observational assessments, further aligning care and accountability with partners involved.
  • Coordinate home visits and in-home observations when necessary, gathering data for chart reviews, staff interviews, and in-depth data analysis.
  • Ensure the effective execution of individualized treatment plans, adjusting approaches based on client progress and feedback.
  • Participate in Interdisciplinary Team Meetings to collaboratively discuss care methods, share insights, and enhance overall client experiences.
  • Complete monthly and quarterly progress notes to keep accurate and organized records of client treatment outcomes and re-assessments.
  • Lead training initiatives that certify staff and team members to competently implement program plans and maintain accurate documentation.
  • Provide proactive follow-up support through additional interviews and home visits, ensuring continuous alignment with client needs.
Patient Population / Client Focus

As a Licensed Mental Health Counselor at Advocate Aurora, you will engage with a diverse population, including adults, youth, couples, and families. Our clientele represents a range of mental health conditions and life challenges, including but not limited to anxiety, depression, trauma, and behavioral issues. Qualifications
  • A Master’s or Doctoral degree in a relevant behavioral health field is required.
