This position is fully remote, allowing our Licensed Clinical Social Workers to work from the comfort of their home environment while connecting with clients throughout California. In your role as a Licensed Clinical Social Worker, your daily responsibilities will include:

• Conducting virtual assessments to evaluate client needs, capabilities, and goals, ensuring that you can tailor your support effectively.
• Designing and implementing individualized care plans that address the psychosocial aspects of end-of-life care, regularly evaluating client progress and adjusting as necessary.
• Providing therapeutic support through scheduled video sessions, actively listening to client concerns, and developing coping strategies to manage emotional distress.
• Acting as an advocate for clients, ensuring they have access to the necessary resources and support systems both within and outside of our organization.
• Collaborating with a team of healthcare professionals remotely, including nurses and physicians, to ensure a comprehensive and coordinated approach to each client’s care.
• Participating in case reviews and team meetings to share insights and enhance the overall quality of care provided.

Your work will particularly focus on clients in hospice care, who are navigating significant life transitions. You will address their emotional, social, and practical needs, while also providing support to family members who may be experiencing grief or challenging dynamics during this period. Understanding the unique psychosocial challenges faced by end-of-life patients will be vital to your success in this role.

To be considered for this position, candidates must meet the following qualifications:

• A Master of Social Work (MSW) degree with an active Licensed Clinical Social Worker (LCSW) certification in the state of California.
• A minimum of two years of direct experience working with clients in a clinical setting.
• Strong technical skills and familiarity with virtual communication tools to ensure effective client engagement.
• Excellent communication skills, demonstrating professionalism and empathy in both verbal and written formats.

While not required, preferred candidates will have experience in hospice or palliative care settings, as well as an understanding of the complexities involved in end-of-life issues. Familiarity with assessments and interventions that are tailored for this population would be advantageous.

The compensation for the Licensed Clinical Social Worker role ranges from USD $115,000 to $120,000 annually, commensurate with experience. In addition to a competitive salary, we offer an attractive benefits package, including:
