• Conduct comprehensive assessments and ongoing evaluations to meet and monitor the needs of clients, ensuring that progress is tracked and adjustments are made to treatment plans as necessary.
• Act as a key communicator among various stakeholders, including supervisors, care staff, family members, and other support networks, to facilitate a holistic approach to client care.
• Develop and provide tentative scheduling for appointments, in-home visits, and observations to assist in effective monitoring and case management.
• Coordinate home visits to perform necessary observations, chart reviews, staff interviews, and data analysis to ensure continuity of care.
• Ensure the effective implementation of individualized program plans and monitor their adherence to promote positive outcomes.
• Participate in interdisciplinary team meetings to discuss client progress, share expert insights, and contribute to collaborative decision-making.
• Complete detailed monthly and quarterly progress notes to document client outcomes and therapeutic milestones.
• Organize and facilitate training sessions to ensure staff competency in implementing program plans and adhering to documentation standards.
• Maintain accurate and accessible records to guarantee essential information is readily available for current and potential stakeholders.
• Serve as a liaison with the broader mental health community to assist with developmental goals and promote collaboration.
• Contribute to the creation of effective training materials aimed at improving programs and client support strategies.

In this role, you will engage with a wide array of clients, including adults and youth facing mental health challenges such as anxiety, depression, trauma, and substance misuse. Your therapeutic expertise will be vital in developing personalized treatment plans, delivering direct counseling services, and ensuring that all clients receive compassionate, tailored support that respects their individual circumstances.

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related discipline is required.
• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas is essential.
• A minimum of two years of relevant counseling experience, particularly in clinical settings, is required.

• Specialization in family therapy, couples therapy, trauma-informed care, or substance abuse counseling is highly preferred.
• A strong understanding of legal and ethical standards guiding the mental health profession is necessary.
• Proficient use of Electronic Medical Records (EMR) software and other relevant technology is favored.
• Demonstrated ability to build rapport with clients from diverse backgrounds and to collaborate effectively within a multi-disciplinary healthcare team.
