Key Responsibilities
  • Conduct comprehensive ongoing assessments of clients to identify their mental health needs, ensuring tailored care approaches.
  • Maintain clear and effective communication with supervisors, direct care staff, family members, and other stakeholders regarding clients' progress, facilitating coordinated care.
  • Draft and provide a tentative schedule for upcoming appointments, home visits, and monitoring observations to optimize client engagement and treatment effectiveness.
  • Coordinate and execute home visits as needed to perform in-home observations, conduct chart reviews, and analyze data to inform treatment plans.
  • Ensure that program plans are being implemented effectively in accordance with clinical standards and organizational goals.
  • Actively participate in Interdisciplinary Team Meetings, contributing insights and collaborating on treatment strategies.
  • Prepare and complete monthly and quarterly progress notes reflecting client advancements and areas for further focus.
  • Organize and provide training for staff members to certify them in the implementation of treatment plans and accurate documentation procedures.
  • Conduct follow-up interviews and home visits as necessary, ensuring continued support for clients' evolving needs.
  • Maintain thorough and accurate records that are accessible to interested parties, ensuring compliance with regulatory requirements.
  • Serve as a liaison to the public and the mental health community, promoting support and resources that align with individual development goals.
  • Assist in the creation of training materials aimed at improving program offerings and resource utilization for clients.
Patient Population / Client Focus

As a Licensed Professional Counselor at Riverwoods Academy, you will work with a diverse client base, ranging from children to adults, facing various mental health challenges.
