Work Environment

This position is primarily remote, allowing you to offer services with flexibility and convenience. Key Responsibilities
  • Conduct comprehensive assessments and ongoing evaluations to effectively address the unique needs of each patient.
  • Maintain clear and open lines of communication with supervisors, family members, and other key individuals involved in a patientโ€™s care, providing regular updates on progress.
  • Develop and provide a tentative schedule of therapy appointments, home visits, and other interventions for effective treatment monitoring.
  • Coordinate home visits and in-home observations to conduct thorough chart reviews and gather insights through interviews and data analysis.
  • Ensure that the behavioral health program plans are executed efficiently and adjusted as needed to align with patient outcomes.
  • Actively participate in interdisciplinary team meetings, offering insights and suggestions for enhancing patient care.
  • Document monthly and quarterly progress notes to keep detailed records that reflect each patientโ€™s journey and treatment progression.
  • Organize and facilitate training sessions to ensure staff proficiency in the implementation of treatment plans and documentation protocols.
  • Provide ongoing follow-up support through interviews, home visits, and observations to assist clients as they navigate their challenges.
  • Maintain accurate and organized records that are easily accessible to authorized individuals involved in patient care.
  • Serve as a liaison with community partners to support broader mental health initiatives and developmental goals.
  • Contribute to the creation of training materials aimed at enhancing the effectiveness of our programs for clients.

This role involves working with a diverse clientele, including both adults and youth who are facing various mental health challenges. Compensation and Benefits
