Job Summary and Responsibilities

Thiis is a remote position requring travel to support enterprise wide Virtual Health Services

As our Virtual Integrated Care Professional, you will deliver comprehensive, coordinated health services remotely, leveraging technology to connect with patients and facilitate seamless care across various disciplines.
Every day you will engage patients through telehealth and other digital tools to conduct assessments, provide education, manage chronic conditions, and coordinate care plans with multidisciplinary teams.
To be successful in this role, you must possess strong communication and clinical assessment skills, comfort and proficiency with virtual healthcare technologies, an ability to work autonomously and collaboratively, and a commitment to patient-centered, integrated care.

  • Serve as a Virtual RN (VIC RN) for 50% of the role, providing direct patient care during implementations including travel
  • Virtually collaborate with interdisciplinary teams, assess patient status, and develop comprehensive care plans
  • Respond to patient issues, educate patients/families, and advocate for their needs while using technology effectively
  • Coach and mentor bedside nurses, focusing on quality, cost containment, and compliance with standards
  • Dedicate 50% to Care Transformation: orienting/mentoring VIC RNs, leading simulations and education, and monitoring workflows/technology
  • Act as a program leader, providing leadership support, troubleshooting barriers, and guiding staff adjustment based on census
Job Requirements

Required Education and Experience

  • Bachelors of Nursing and 5 years of overall acute care nursing experience
  • 3 years of nursing preceptor experience
  • 2 years in a nursing leadership role
  • Basic Life Support - CPR
  • Virtual care and critical care experience is preferred
