Your Role

The Registered Nurse Case Manager is a key member ofย AptiHealth'sย Case Management and Medication Management programs, supporting both patients and providers across theย careย continuum. This role assesses patient needs, develops and implementsย careย plans, coordinatesย careย and medications, triages urgent patient concerns, and supports clinical operations in collaboration with the multidisciplinaryย careย team.ย The RNย Caseย Manager ensures high-quality, patient-centeredย careย while maintaining compliance with company policies and regulatory requirements.ย 

Responsibilities

  • Complete case management assessments and developย careย plans
  • Collaborate with theย careย team toย identifyย patient needs and coordinateย care
  • Document all patient interactions andย careย coordination activities
  • Triage patient calls and electronic communications, escalating concerns as appropriate
  • Support prescribers with prior authorizations, refills, pharmacy coordination, and scheduling based on clinical needย 
  • Conduct medical reviews and gatherย additionalย patient history as neededย 
  • Obtain and manage required patient consents and releases of informationย 
  • Educate and onboard patients intoย AptiHealth'sย Medication Management Programย 
  • Assess patient response to treatment and medications
  • Connect patients with community resources and support services to address social determinants of healthย 
  • Conduct proactive telephonic outreach to patients based on clinical needs

Requirements

  • Active Registered Nurse (RN) license in New Yorkย requiredย ย 
  • Graduate of an accredited nursing program; BSN preferredย ย 
  • Certified Case Manager (CCM) certification preferredย ย 
  • Behavioral health or psychiatricย careย experience strongly preferredย ย 
  • Experience supportingย careย coordination and case management workflowsย ย 
  • Proficiencyย with Electronic Medical Record (EMR) systems and Microsoft Office applications
  • Strong organizational, communication, and clinical triage skillsย ย 
  • Ability to work collaboratively within a multidisciplinaryย careย teamย ย 
  • Comfortable working in a fast-paced, patient-centered virtualย careย environment
