As a Licensed Mental Health Therapist, you will play a crucial role in the therapeutic journey of your clients. Your key responsibilities include:

• Conducting comprehensive psychosocial assessments to diagnose and treat a range of mental, emotional, and behavioral disorders, adjusting care plans as needed to align with the treatment progress.
• Developing individualized treatment plans in consideration of each client’s unique circumstances and collaborating closely with families and caregivers to facilitate support.
• Providing crisis intervention and connecting clients to necessary community resources, treatment programs, and support groups, ensuring they receive holistic support for their well-being.
• Regularly reviewing and updating care plans in conjunction with fellow healthcare providers, promoting continuity of care that lays the groundwork for successful outcomes.
• Engaging in multidisciplinary team meetings to strategize on client care and discharge planning, thereby ensuring clients transition smoothly from therapy to independent living.
• Documenting patient progress comprehensively in our case management system, ensuring compliance with all applicable local, state, and federal standards.
• Advocating for clients' needs with both internal teams and external services to ensure effective communication and coordination of care.
• Utilizing the telehealth platform to schedule and conduct therapy sessions, allowing for flexibility in your caseload management.

To be considered for this position, candidates must possess the following qualifications:

• A Master’s Degree in Social Work or a related field, with accreditation verification required.

Rocky Meadow Counseling offers a competitive salary ranging from $90,000 to $100,000 annually, commensurate with experience and qualifications. Additional benefits include:
