Join a leading Home Health team providing compassionate, patient-centered care to adult and geriatric patients throughout the Central Metro E Chicago coverage area. This role offers flexibility with a primarily remote workflow while conducting in-home patient visits within the assigned territory.

  • Expected Visits: 5-6 patients per day

  • Current Team Census: Approximately 200 patients

Traveler can expect approximately 2 days onsite in Oak Brook, IL for orientation and onboarding. After orientation, the role is fully remote/field-based within the coverage area.


Candidate RequirementsRequired Experience
  • Minimum 2 years of Home Health PT experience

  • Adult and geriatric patient experience required

  • First-time travelers considered for the right candidate

  • 1-year experience candidates considered if recent Home Health experience

Required Licensure & Certifications
  • Active Illinois PT License

  • BLS (AHA)

  • Valid Driver's License

  • Active Auto Insurance

  • Reliable Transportation


Patient Population
  • Adult and geriatric general Med/Surg patients

  • Assisting patients with ADLs (Activities of Daily Living)

  • Adaptive equipment training and education
