• Conduct thorough and comprehensive assessments of clients to identify social, emotional, and environmental challenges that may be impacting their well-being.
• Develop and implement personalized service plans that define goals, interventions, and methods for monitoring progress over time.
• Provide evidence-based individual and family counseling sessions, utilizing various therapeutic modalities to support clients in overcoming challenges.
• Offer crisis intervention services, performing risk assessments and creating safety plans for clients experiencing acute distress.
• Collaborate effectively with clients to connect them to vital community resources, ensuring their needs are met holistically.
• Engage with community agencies and organizations to coordinate comprehensive services and provide advocacy on behalf of clients.
• Participate in community outreach initiatives to enhance awareness of available services and promote agency engagement.
• Attend case conferences and team meetings regularly to share insights, best practices, and collectively strategize on client support.

As a Remote Care Specialist, you will cater your practice to a diverse client population, many of whom are experiencing significant hardship. By using a trauma-informed approach, you will help clients navigate through complex life situations, facilitating not only their immediate needs but also their long-term mental health and stability.

• A Master’s degree in Social Work from an accredited institution is required.
• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong knowledge base in social and economic justice principles, community resources, and trauma-informed care practices.
• Excellent communication skills with a demonstrated ability to engage effectively with a diverse clientele.
• High levels of cultural competence and the ability to approach problem-solving creatively and empathetically.

• Previous experience working in a social service agency or mental health setting is highly desirable.
• Proficiency in case management systems or electronic health record platforms related to client data and reporting.
• Familiarity with evidence-based therapeutic modalities and crisis intervention techniques.
