Job description

The Clinical Growth Lead is a strategic, client-facing role responsible for driving growth across existing accounts while supporting new client onboarding and expansion within the current territory. This individual bridges clinical expertise with business development, ensuring partner practices maximize the value of care management programs such as Chronic Care Management (CCM), Remote Therapeutic Monitoring (RTM), Remote Patient Monitoring (RPM) and related services.

This role plays a critical part in strengthening client relationships, increasing patient enrollment, optimizing program performance, and identifying opportunities for service expansion and new client acquisition.

Key Responsibilities

1. Client Growth & Account Management

  • Serve as the primary point of contact for assigned partner practices
  • Build strong, trusted relationships with providers, administrators, and clinical staff
  • Identify and execute opportunities to expand service offerings (RPM, CCM, PCM, RTM, etc.) within existing accounts
  • Conduct regular performance reviews, presenting data on enrollment, clinical outcomes, and program impact
  • Ensure client satisfaction, retention, and long-term partnership growth
  • Schedule and conduct onsite meetings with the assigned partner practices

2. Enrollment Strategy & Management

  • Partner with Enrollment Team Lead to drive patient enrollment growth and engagement
  • Monitor key enrollment metrics and proactively address gaps or performance issues
  • Train and guide practice staff on best practices for patient identification and enrollment workflows
  • Continuously optimize processes to improve efficiency, compliance, and patient outcomes

3. New Client Onboarding

  • Lead onboarding for new partner practices, ensuring a smooth and efficient implementation process
  • Coordinate cross-functionally with clinical, operations, and billing teams
  • Provide training and education on program structure, workflows, and expectations
  • Establish clear success metrics and timelines for program launch and growth
  • Ensure early adoption and momentum within the first 90 days

4. Territory Growth & Business Development

  • Identify and pursue opportunities to bring in new clients within the current territory
  • Leverage existing relationships, referrals, and market knowledge to generate leads
  • Support sales conversations with clinical expertise and program knowledge
  • Collaborate with leadership on territory growth strategy and targeting
  • Travel will be required for this role with quarterly visits to each client plus onsite support during implementations

Qualifications & Expertise

  • Clinical Background Required (RN, RT, or similar), with experience in pulmonary, sleep medicine, or chronic disease management preferred
  • Strong knowledge of Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and/or Principal Care Management (PCM) programs
  • Demonstrated experience in account management or client-facing roles
  • Working knowledge of medical billing, reimbursement models, and care management program economics
  • Proven ability to contribute to or support sales and business development efforts
  • Strong analytical mindset with ability to interpret data and translate into actionable strategies
  • Excellent communication and relationship-building skills
  • Highly organized with the ability to manage multiple accounts and priorities
