Key Responsibilities
  • Deliver individualized assessment and clinical intervention services to clients suffering from severe mental health disorders through virtual platforms, ensuring an accessible and supportive environment.
  • Create and implement integrated care plans that address the unique needs of each client while facilitating access to necessary resources and services.
  • Conduct evidence-based, developmentally appropriate assessments to tailor interventions and monitor client progress, adjusting treatment plans as necessary.
  • Provide direct services including individual, group, and family therapy sessions, with an emphasis on empowerment and recovery.
  • Collaborate with a diverse team of healthcare providers, ensuring comprehensive care through consultation and information sharing about client progress and needs.
  • Identify and report any potential safety risks, proactively creating a safe and secure remote environment for all clients.
  • Engage in crisis intervention as needed, utilizing de-escalation techniques and providing appropriate referrals.
  • Maintain accurate and timely documentation of client interactions, treatment plans, and progress notes, ensuring compliance with organizational standards.
  • Participate in team meetings and committees to enhance service delivery and contribute to quality improvement initiatives.
  • Provide on-call coverage during specified hours to ensure continuous support for clients as needed.

Qualifications
  • Active LCSW licensure in the state of Tennessee.
