Water Edge Counseling Services is seeking a dedicated and innovative Licensed Clinical Social Worker (LCSW) to join our growing team. Key Responsibilities
  • Conduct initial patient assessments to evaluate behavioral health symptoms, risk factors, and care needs.
  • Develop, manage, and implement personalized care plans tailored to each patient's individual behavioral health needs.
  • Provide virtual psychotherapy sessions, lasting 45-50 minutes, utilizing a variety of therapeutic techniques such as mindfulness and motivational interviewing.
  • Coordinate patient care across Water Edge Counseling Services and partner health systems, ensuring seamless communication and collaboration.
  • Regularly re-evaluate and adjust treatment plans based on patient progress and emerging needs, ensuring alignment with evidence-based practices.
  • Document all client interactions accurately and in a timely manner, maintaining high standards of confidentiality and compliance.
  • Promote patient engagement by setting achievable goals and encouraging adherence to treatment plans.
  • Make independent clinical judgments on treatment adjustments based on comprehensive evaluations and ethical practice standards.
Patient Population / Client Focus

You will primarily work with patients who are navigating complex behavioral health challenges, often interconnected with cardiac rehabilitation. This unique focus allows you to utilize your clinical skills to effect significant, positive change in their lives, helping them achieve both mental and physical health recovery.

Qualifications
  • A valid and unrestricted LCSW license in New York State.
  • A completed Bachelor’s degree in Social Work or a related field.
  • Strong clinical skills in psychotherapy, patient assessment, and care planning.
  • Innovative thinking with a willingness to challenge traditional rehabilitation models.
  • Ability to work collaboratively within a diverse healthcare team.
Preferred Experience
