Urban Arc Psychiatry Clinic is excited to announce an opening for a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity. This role is crucial to providing high-quality psychosocial support to patients and their families, helping them navigate through the complexities of hospitalization, illness, and life transitions. As an LCSW, you will conduct comprehensive assessments, develop intervention plans, and facilitate resources to promote healthy coping mechanisms in a hospice center environment.

Working in a hospice center, you'll be focused on providing emotional and psychological support to individuals facing challenging life circumstances, making your role impactful during significant transitions in their lives.

• Conduct thorough psychosocial assessments of patients and their families to identify emotional, social, and environmental strengths and challenges associated with their illness and treatment plans.
• Develop, implement, and adjust individualized care plans, utilizing diverse therapeutic modalities, including crisis intervention and both brief and long-term therapies.
• Work collaboratively with healthcare team members, including physicians, nurses, and support staff, to ensure comprehensive patient care and facilitate communication among all parties involved.
• Lead discharge planning efforts, coordinating with various healthcare providers and third-party payers to create effective post-hospitalization plans that support patient transitions.
• Provide education and counseling support to patients and families, equipping them with strategies to cope with complex emotions related to their diagnosis and treatment journey.
• Clinically evaluate oncology patients for mental health challenges such as depression and anxiety, delivering appropriate short-term interventions as needed.
• Be available for on-call rotations, ensuring that immediate support is available for after-hours needs.
• Serve as a resource for other staff members, offering guidance in psychosocial oncology practices and sharing expertise in educational settings.
• Engage with the community through outreach efforts, enhancing the support systems for cancer survivors and promoting mental health awareness.

Your primary focus will be on patients facing serious health conditions and their families, particularly within the hospice care framework. This role emphasizes compassionate communication and empathetic practice as you support individuals navigating profound life changes and emotional distress.

• Master's degree in Social Work (MSW) from an accredited institution.
• Current and valid Illinois LCSW licensure.
• Proven ability to conduct comprehensive psychosocial assessments and develop effective treatment plans.
• Strong interpersonal and communication skills, with a focus on building therapeutic relationships.

• Experience in hospice care, oncology, or working with chronically ill populations is highly desirable.
• Familiarity with electronic health record (EHR) systems.
• Previous experience in multidisciplinary healthcare teams.
• Demonstrated success in patient and family counseling.

This full-time role offers a competitive salary range of USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
