• Provide expert clinical consultation to fellow clinical staff regarding complex mental health issues across diverse patient categories, including elderly individuals and those battling substance use disorders.
• Prepare and communicate vital patient information, including comprehensive social histories to enhance understanding among staff and involved healthcare providers.
• Lead the coordination of individual patient care plans, focusing on actionable recommendations and facilitating decisions that impact clients’ home situations and overall well-being.
• Engage in treatment planning and regularly review cases with providers to optimize therapeutic outcomes and ensure a multidisciplinary approach to mental health care.
• Perform additional duties as assigned, contributing to the overall effectiveness and responsiveness of our mental health services.

At Talbott Mental Healthcare Center, you will work with a vast and varied clientele. Our patients include elderly individuals facing age-related mental health issues, as well as clients dealing with substance use disorders. • A Master’s Degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling is required.
• Must possess valid independent licenses such as Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).
• Proven ability to efficiently document progress notes promptly following client sessions, ensuring accurate and organized patient records.

• Familiarity with electronic health record (EHR) systems and experience managing virtual therapy sessions is preferred.
• A background in engaging with multidisciplinary teams and participating in case discussions to improve client outcomes is highly valued.
