• Conduct comprehensive assessments of clients to identify their mental health needs and develop tailored treatment plans.

• Collaborate closely with healthcare professionals, including physicians and nurses, to ensure a holistic and integrated approach to client care.
• Maintain meticulous and accurate records, including progress notes and treatment plans, to document each client’s journey and ensure continuity of care.
• Engage in regular supervision and professional development activities, staying updated on best practices in clinical social work to enhance treatment effectiveness.

As a Licensed Clinical Social Worker at Health Bridge Psychotherapy Center, you will primarily serve clients in hospice care, including terminally ill patients and their families. Your role will focus on addressing the emotional, psychological, and social challenges associated with chronic illness, as well as providing grief counseling and support to loved ones coping with impending loss.

• Master's degree in Social Work from an accredited institution, with a focus on clinical practice.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Arizona.
• A minimum of three years of clinical experience in a mental health or outpatient setting, preferably with a focus on hospice or palliative care.
• Exceptional skills in assessment, diagnosis, and therapeutic intervention, with a strong knowledge of evidence-based practices.
• Outstanding communication and interpersonal skills to effectively connect with clients and their families during sensitive times.

• Experience working within a hospice or palliative care environment, with an emphasis on end-of-life counseling.
• Familiarity with client management systems and electronic health records (EHR).
• Training in grief and loss counseling or similar therapeutic modalities.
