We provide expert diabetes education and clinical management for Medicare patients, delivered by a comprehensive team of specialized Registered Nurses, Registered Dietitians, and Accountability Coaches.
Our model integrates high-level clinical expertise with real-time data from cellular-connected glucometers and advanced CGM management. By bridging the gap between physician visits with proactive, data-driven support, we deliver life-changing outcomes for our members.
We are seeking an experienced Registered Dietitian (RD) and Certified Diabetes Care and Education Specialist (CDCES) passionate about remote care delivery. In this role, you will offer diabetes education and management, medical nutrition therapy, and personalized nutritional guidance for our members. You will partner with a multidisciplinary care team to ensure members receive expert clinical attention and education to lead healthier lives.
Contractors are paid on a monthly basis following Net 30 payment terms.
