Overview

We’re hiring a Community Partnerships Manager (Field Sales Representative) to drive referral growth and expand community relationships across our established Greater Albuquerque markets (Central and Southwest), while developing our newer Northwest New Mexico territories. We are a Home Health Services organization backed by a $2B national organization serving all 50 states, with a focus on Medicaid recipients, self selected care, older adults, and individuals with disabilities.


This is a field sales and business development role for someone who enjoys building relationships, creating opportunities from scratch, and being accountable to clear performance metrics. You’ll spend approximately 80% of your time in the field meeting with healthcare providers, community centers, nonprofit organizations, and other referral partners to generate new patient referrals and expand access to long term care in the comfort of patients’ own homes.


Territory & Travel

The initial territory includes Southern Albuquerque / Los Lunas, Gallup, and Farmington. Travel to Gallup and Farmington is expected approximately twice per month while helping develop the Northwest New Mexico market.


Responsibilities

• Drive referral generation, patient admissions, and revenue growth across the assigned territory

• Build relationships with healthcare providers, community organizations, Medicaid populations, developmental disability resources, and alternative referral channels

• Execute field sales activities including prospecting, 1:1 meetings, presentations, community events, open houses, and partnership development

• Independently manage territory strategy, scheduling, pipeline, CRM activity, follow ups, and performance against established KPIs

• Help develop the Gallup and Farmington markets, traveling to Northwest New Mexico approximately twice per month

• Partner with Operations and leadership to ensure sales growth aligns with staffing capacity and service availability


Ideal Background

• 1-5 years of outside sales or business development experience across any industry (B2B).

• Highly Desired: sales exposure within healthcare, community outreach, pharmaceuticals, laser diagnostics, med device, etc. Experience with Medicaid, developmental disability, or Native American communities is also highly valued.

• Self motivated, resilient, and comfortable with cold outreach and developing new relationships.

• Strong territory planning, go-to-market instincts, interpersonal communication, relationship management, and organizational skills.

• Comfortable working autonomously and being measured against revenue, admission, referral, and activity KPIs.

• Bachelor’s degree preferred.
