Weekend Coverage LPN - Remote Patient Monitoring
Pay: $25.00-$26.00/hour
Location: Remote
Schedule: Wednesday-Sunday | 9:00 AM - 5:30 PM EST

If you're an LPN who values autonomy, is comfortable with technology, and wants to contribute to proactive, preventive care, this is an opportunity to be part of a growing, forward-thinking team.
What You'll Do
You will play a key role in monitoring and supporting patients managing chronic conditions-engaging them consistently and coordinating care when it matters most.

  • Monitor RPM dashboards and respond to alerts (blood pressure, glucose, weight, etc.)
  • Call patients regularly to assess symptoms and reinforce device usage
  • Coordinate follow-up with NPs, primary care providers, and caregivers
  • Document patient interactions in EMR and internal systems
  • Troubleshoot device issues to ensure patient compliance
  • Serve as a primary point of contact for a panel of patients
  • Support patient enrollment and onboarding as needed
  • Collaborate daily with NPs, intake team, and support staff

What We're Looking For

  • Interest in preventive care and patient engagement
  • Ability to work effectively in a fast-paced, structured environment
