Job description

The Home Health RN Case Manager plays a crucial role in providing comprehensive nursing care to patients with chronic illnesses and post-acute conditions in their home environment. This position involves conducting thorough patient assessments, developing personalized care plans, coordinating with interdisciplinary teams including physicians and therapists, and managing medications to ensure optimal patient outcomes. With a focus on home visits and moderate travel within the local area, the nurse supports a diverse caseload while educating patients and documenting care accurately using electronic medical records.

Responsibilities

  • Conduct patient assessments and develop individualized care plans
  • Coordinate care activities across interdisciplinary teams, including physicians and therapists
  • Manage patient medications and monitor adherence
  • Provide patient education tailored to individual health needs
  • Document clinical information accurately using electronic health records
  • Perform home visits to deliver direct nursing care and evaluate patient progress
  • Collaborate on wound care and conduct Start of Care (SOC), Recertification (Recerts), and Resumption of Care (ROC) assessments

Preferred Qualifications

  • Minimum of 1 year experience in home health nursing
  • Current Registered Nurse (RN) License
  • Strong clinical assessment and care coordination skills
  • Proficient in patient education and communication
  • Experience with Electronic Health Records (EHR), preferably Kinnser EMR
  • Familiarity with wound care management
  • Effective problem solving and time management abilities
