As a Licensed Clinical Social Worker on our team, you will be responsible for a range of clinical duties aimed at improving patient outcomes. Your specific responsibilities will include:

• Conducting comprehensive clinical assessments to identify patients' needs and strengths, utilizing standardized assessment tools.
• Developing individualized treatment plans informed by established best practices and evidence-based interventions designed to address the complex emotional and psychological needs of patients and their families.
• Implementing a variety of therapeutic methodologies, including cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and other solution-focused modalities appropriate for hospice care.
• Documenting all clinical interactions and maintaining accurate, timely records in compliance with HIPAA regulations to ensure confidentiality and quality of care.
• Engaging in collaborative team meetings with other professionals, such as psychiatrists and nurse practitioners, to discuss treatment progress and refine care strategies.
• Providing psychoeducation and emotional support to patients and their families, guiding them through the challenges associated with hospice care and available community resources.
• Continuously reviewing and integrating the latest research findings into your practice to enhance the care provided.

Your primary focus will be on a diverse patient population facing terminal illnesses, ranging from cancer and heart disease to neurological conditions. Preparing families for end-of-life decisions while fostering an atmosphere of emotional support is critical. This role demands sensitivity and a deep understanding of grief and loss, requiring you to navigate both individual and family dynamics delicately.

• A Master's Degree in Social Work from an accredited institution, reflecting a strong academic foundation in clinical practice.
• A valid and active license to practice as a Licensed Clinical Social Worker (LCSW) or Licensed Specialist Clinical Social Worker (LSCSW) in the state of Kansas.
• A minimum of two years of clinical experience, specifically in providing psychotherapy services within a healthcare or hospice setting.
• Proficiency in evidence-based, solution-focused therapeutic interventions and an understanding of hospice care dynamics.
• Exceptional communication, interpersonal, and relationship-building skills to effectively engage with patients, families, and colleagues alike.
