• Conduct in-depth assessments of seniors' mental, emotional, and social needs to create individualized care plans that align with their goals.
• Provide counseling services to clients and their families, guiding them through challenges such as transitions into assisted living, health crises, or end-of-life issues.
• Advocate for clients to secure access to essential community resources, including healthcare services, financial assistance, and social support systems.
• Coordinate with healthcare professionals, caregivers, and other service providers to deliver comprehensive care and ensure clients' needs are holistically met.
• Monitor clients’ progress through regular check-ins and follow-up assessments, adjusting care plans as necessary based on changing circumstances or feedback.
• Facilitate communication among family members, fostering understanding and cooperation in care processes, and resolving potential conflicts.
• Maintain meticulous documentation of client interactions, assessments, care plans, and adherence to compliance standards within EHR systems.

You will primarily work with seniors, addressing a wide range of issues including mental health challenges, chronic illness management, and social isolation. Your role is critical in evaluating not only the physical health of clients but their emotional and social well-being, ensuring a balanced approach to their care. • A Master’s degree in Social Work from an accredited institution, ensuring a robust foundation in clinical practice and theory.
