This position is entirely remote, allowing for flexibility while still fostering a collaborative and supportive atmosphere among colleagues. • Conduct timely and respectful patient assessments, addressing the unique needs and concerns of each individual and their families in accordance with their scheduled appointments.
• Provide therapeutic interventions that comply with ethical and legal standards set by relevant regulatory bodies. Documentation of all sessions is to be completed within 72 hours to maintain thorough records that align with professional norms.
• Collaborate with support staff to ensure an efficient and full caseload, proactively scheduling appointments and managing follow-up consultations effectively.
• Utilize evidence-based practices to develop treatment plans, supporting patients through various psychosocial challenges associated with end-of-life care.
• Engage in continuous learning to enhance your skill set and stay updated on new developments within the field of social work and hospice care.

Your primary focus will be on patients receiving hospice care, which includes individuals with terminal illnesses and their families. Understanding the multifaceted emotional, psychological, and social implications of illness and death will be an integral part of your role. You will provide support during a critical phase of life, assisting patients in navigating grief, fear, and loss while promoting dignity and quality of life.

• A Master's Degree in Social Work from an accredited institution.
• Active and unrestricted Licensed Independent Clinical Social Worker (LICSW) credential.
• A minimum of 2 years of experience in a behavioral health or social services setting, with a focus on case management and direct therapeutic services.
• Strong conflict resolution skills, enabling you to handle sensitive discussions effectively with patients and their families.
• Proficient in conducting psychosocial assessments, including interview techniques and crisis intervention strategies.

• Experience in hospice, palliative care, or a related healthcare environment is highly desirable.
• Familiarity with electronic health records (EHR) systems for efficient documentation and communication.
• Proven ability to work collaboratively within a multidisciplinary team, contributing to comprehensive patient care plans.
