• Conduct thorough clinical assessments of patients in hospice care, developing personalized treatment plans tailored to their unique needs and circumstances using telehealth technologies.
• Utilize evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), specifically adapted for a remote environment to foster emotional resilience in patients.
• Maintain diligent and accurate clinical documentation within secure electronic health record (EHR) systems, ensuring adherence to HIPAA regulations and organizational standards.
• Collaborate with a multidisciplinary team of mental health professionals, including psychiatrists and psychologists, to provide comprehensive support via virtual meetings, enhancing patient care outcomes.

• A Master's degree in Social Work from an accredited institution is essential.
