Key Responsibilities
  • Conduct comprehensive assessments and regular evaluations to determine the mental health needs of clients, tailoring treatment plans accordingly.
  • Facilitate effective communication among clients, families, and interdisciplinary team members to provide clear updates on progress and treatment goals.
  • Document and share a structured plan with supervisors, detailing appointments, home visits, and observation sessions to evaluate client progress.
  • Arrange and conduct necessary home visits for client observations, data analysis, and insight gathering, ensuring treatment plans are appropriately implemented.
  • Participate in interdisciplinary team meetings and collaborate with other healthcare professionals to ensure the delivery of integrated services.
  • Prepare and file monthly and quarterly progress reports, ensuring that client records are accurately maintained and accessible to authorized personnel.
  • Provide training sessions to colleagues on program execution and documentation, enhancing the team’s effectiveness in service delivery.
  • Execute follow-up interviews and observations to ensure ongoing alignment of client needs with provided care.
Patient Population / Client Focus

You will primarily work with a diverse population of individuals, including children, adolescents, adults, and families facing various mental health challenges. This role allows for the opportunity to specialize in areas of family therapy, trauma-informed care, or substance use counseling, which will help address a wide array of client issues and life circumstances.

Qualifications
  • A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
  • Possession of an active and unrestricted licensure as a Professional Counselor (LPC) in the state of Oregon.
  • A minimum of two years of experience in a clinical counseling setting, demonstrating a strong competency in delivering individual therapy and group sessions.
  • Familiarity with telehealth practices, electronic medical record (EMR) systems, and relevant therapy technologies is essential.
  • A profound understanding of ethical and legal standards in mental health counseling.
  • Exceptional interpersonal and communication skills to build rapport and trust with a varied clientele.
