As a member of our Community Health Clinic, you will collaborate with a team of dedicated social workers, therapists, and clinicians who share a common goal of providing high-quality care and support. • Conduct thorough evaluations and assessments of children to determine their developmental needs.
• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP) to promote child and family welfare.
• Monitor and evaluate outcomes collaboratively as a member of the interdisciplinary team, ensuring continuous improvement in service delivery.
• Establish strong, trusting relationships with children and their families to foster a healthy learning environment.
• Provide ongoing support and education to families, equipping them with tools to enhance their child’s development through evidence-based practices.
• Utilize the coaching model effectively and apply Natural Learning Environment Practices during interactions with families.
• Ensure compliance with all applicable Federal and State regulations as well as organizational policies and standards.
• Adhere to best practices in safeguarding the health and safety of all individuals served.

The selected LCSW will primarily work with young children, specifically infants and toddlers experiencing developmental delays or disabilities. The role requires sensitivity and flexibility to engage effectively with diverse family dynamics, ensuring that interventions are tailored to each child's unique needs and family priorities. Our services not only focus on the child but also empower families with knowledge and tools to support their child’s growth and well-being.

• Master's degree in Social Work or a Doctorate of Social Work from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in Illinois.
• A valid driver's license for potential travel to clients' homes or community meetings.
• Ability to adapt to various learning styles and cultural backgrounds, addressing the specific needs of each child and family.

• A minimum of one year of experience working in early intervention or with young children experiencing developmental challenges.
• Proficiency in Natural Learning Environment Practices and evidence-based therapeutic strategies.
