As a Licensed Clinical Social Worker at Safe Ground, your core responsibilities will include:

• Collaboratively developing individualized treatment and discharge plans with interdisciplinary healthcare teams through comprehensive psychosocial assessments.
• Conducting thorough evaluations and timely interventions tailored to the unique circumstances and needs of each patient.
• Providing empathetic counseling and crisis intervention, addressing emotional and behavioral challenges faced by patients undergoing detox treatment.
• Facilitating communication and coordination with patients’ families or support systems regarding treatment plans and discharge processes, ensuring all parties are informed and involved.
• Maintaining meticulous documentation of clinical encounters and progress notes as per department policy, which contributes to continuity of care and regulatory compliance.

Your work primarily focuses on individuals undergoing detoxification from substances, who may present with a range of emotional and behavioral complexities. To be considered for the Licensed Clinical Social Worker position, candidates must meet the following qualifications:

• Current and active license as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.

While not mandatory, candidates with additional experience in detoxification settings, crisis intervention, and family systems therapy will be viewed favorably.
