Gotham Enterprises Ltd is seeking a compassionate and skilled Licensed Clinical Social Worker (LCSW) to join our dedicated team at our Community Center located in Pikeville, Tennessee. Work Environment

This role is predominantly remote, which allows for flexibility in working hours while still ensuring our seniors receive the finest care possible. You will be part of a multidisciplinary team, collaborating seamlessly with healthcare professionals, caregivers, and other social workers. Though the standard work hours are Monday to Friday, 8:00 AM to 4:30 PM, occasional evening or weekend hours may be necessary to respond to the needs of clients. On-call availability may also be required to address urgent client situations, ensuring continuous support and care.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments for seniors, evaluating their emotional, social, and mental health needs.
  • Develop and implement individualized care plans that align with the unique circumstances and preferences of each client.
  • Provide counseling services to seniors and their families to assist them in navigating challenges related to aging and health transitions.
  • Act as an advocate for your clients by facilitating access to community resources and services that support their well-being.
  • Collaborate with healthcare professionals, caregivers, and families to coordinate care and address any safety concerns in clients' living environments.
  • Monitor client progress through regular home visits, assessing changes in their emotional and social circumstances.
  • Educate clients and families about available resources, social programs, and support networks that can improve their quality of life.
  • Document all client interactions, assessments, and care plans accurately in our electronic health record (EHR) system.
  • Participate in ongoing professional training to stay abreast of the latest developments in social work practices and policies.
Patient Population / Client Focus

You will primarily serve senior clients who may be dealing with conditions related to aging, including mental health issues such as depression and anxiety, chronic health conditions, and life transitions. Your role will be crucial in addressing the psychosocial needs of this population, encouraging their autonomy, and enhancing their social engagement.

Qualifications
  • A Master's degree in Social Work from an accredited institution.
  • Possession of a current and valid LCSW license for the state of Tennessee.
  • At least two years of relevant experience in social work, preferably in a community setting focused on geriatric care.
  • Strong understanding of social work principles, aging-related issues, and community resources available to seniors.
  • Excellent skills in assessment, counseling, and problem-solving.
  • A demonstrated ability to empathize with seniors and their families, building rapport quickly.

This role offers a competitive salary range of USD 60,000 to 100,000 per year, commensurate with experience and qualifications.
