Key Responsibilities
  • Manage a caseload of clients, establishing rapport and nurturing positive working relationships while adhering to professional ethics and maintaining appropriate boundaries.
  • Conduct regular client meetings to assess progress, stability, and readiness for discharge when appropriate, ensuring a person-centered approach.
  • Develop, implement, and continuously monitor individual treatment plans tailored to each client's specific needs and goals.
  • Maintain accurate and up-to-date documentation, including progress notes and treatment records, in alignment with organizational policies and procedures.
  • Execute intake assessments to gather comprehensive client histories and baseline information.
  • Facilitate weekly individual therapy sessions utilizing evidence-based practices tailored to each client's unique circumstances.
  • Engage in ongoing individual supervision and professional growth experiences.
  • Collaborate with other healthcare providers to ensure a multidisciplinary approach to client care.

Qualifications

  • Competence in conducting in-depth intake assessments to inform treatment planning.
