About the Opportunity

A prominent home-based care organization in the Greater Boston area is seeking a Board-Certified Medical Director to provide clinical oversight, strategy, and direct patient care within their high-quality home-based primary care program. This key leadership role balances clinical delivery with administrative leadershipโ€”ideal for an experienced physician passionate about advancing innovative, patient-centered care for homebound adult and geriatric populations.

Key Responsibilities

  • Provide clinical direction, quality management, and administrative leadership for the home-based primary care team.
  • Deliver compassionate, comprehensive primary care directly to patients in their homes across the Greater Boston footprint.
  • Mentor and oversee advanced practice providers (NPs/PAs), clinical staff, and multidisciplinary care management teams.
  • Develop, refine, and enforce clinical protocols, quality assurance standards, and clinical outcome metrics.
  • Collaborate with health systems, ACOs, and community partners to optimize care coordination and reduce avoidable hospital readmissions.
  • Ensure regulatory compliance and maintain efficient documentation within the EHR system.

Qualifications

  • Board Certified in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • Active, unencumbered Massachusetts Medical License (MD or DO).
  • Active Massachusetts DEA and Controlled Substance Registration (CSR).
  • Minimum 3โ€“5 years of clinical primary care experience; prior leadership or administrative experience in home-based primary care, house calls, or value-based care models highly preferred.
