Key Responsibilities
  • Provide individual, group, and family therapy through teletherapy platforms, addressing emotional and psychological needs specific to hospice care.
  • Conduct comprehensive psychosocial assessments to identify patient needs and develop tailored treatment plans that include therapeutic goals.
  • Utilize evidence-based interventions such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and trauma-informed care methodologies to foster therapeutic engagement.
  • Deliver crisis intervention services when needed, ensuring clients have immediate support during acute emotional distress.
  • Maintain accurate and confidential documentation through our Electronic Health Record (EHR) system, adhering to HIPAA regulations.
  • Collaborate with a multidisciplinary team to provide holistic care that includes physical, emotional, and psychosocial support.
  • Participate in regular clinical meetings and case reviews to share insights and enhance treatment protocols.

Patient Population / Client Focus

Qualifications
