• Provide individual, group, and family therapy through teletherapy platforms, addressing emotional and psychological needs specific to hospice care.
• Conduct comprehensive psychosocial assessments to identify patient needs and develop tailored treatment plans that include therapeutic goals.
• Utilize evidence-based interventions such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and trauma-informed care methodologies to foster therapeutic engagement.
• Deliver crisis intervention services when needed, ensuring clients have immediate support during acute emotional distress.
• Maintain accurate and confidential documentation through our Electronic Health Record (EHR) system, adhering to HIPAA regulations.
• Collaborate with a multidisciplinary team to provide holistic care that includes physical, emotional, and psychosocial support.
• Participate in regular clinical meetings and case reviews to share insights and enhance treatment protocols.
• Ensure compliance with all state licensing regulations and uphold high professional standards in practice.

In this role, you will focus on supporting patients and their families dealing with terminal illnesses. • Active LCSW license in the state of California, with a clean record of practice.
• Master’s Degree in Social Work (MSW) from an accredited program that meets the educational requirements for licensure.
• A minimum of 2 years of clinical experience following licensure, with a preference for experience in hospice or palliative care settings.
• Proficiency in evidence-based therapeutic interventions, with a strong foundation in CBT, DBT, and trauma-focused therapy.
• Exceptional communication skills, both verbal and written, are essential for effective remote counseling.

• Experience providing telehealth services, demonstrating adaptability to virtual platforms and remote counseling techniques.
• Familiarity with Electronic Health Record (EHR) systems, specifically in clinical documentation and client management.
• Skills in case management, crisis intervention, and interdisciplinary team collaboration.
