Job Description:
Job Summary
Responsible for providing patient-centered care as part of a multidisciplinary ambulatory team by performing assessments, direct care, and simple procedures within the LPN scope of practice. Works under RN supervision to implement and evaluate nursing care plans and ensure patient satisfaction. Delegates tasks to nursing assistants and adheres to regulatory standards. Fosters collaboration to enhance patient outcomes and experiences.
Essential Functions
• Assesses patients, conducts screenings, and documents findings using standard scales and protocols.
• Prioritizes patient needs, collaborates with the healthcare team, and develops individualized care plans.
• Provides patient care and treatments, including medication administration, procedures, and disease management under appropriate supervision.
• Delivers age-appropriate, culturally sensitive, and ethical care while maintaining patient privacy and confidentiality.
• Educates patients and families on care, treatment, and risk factors, ensuring understanding and documenting education provided.
• Maintains accurate, timely medical records and documentation, and utilizes critical thinking and evidence-based practice.
• Delegates tasks appropriately, provide guidance to float employees, and assists with office nursing duties as needed.
• Participates in quality and safety initiatives, team collaboration, professional development, and maintains required licensure and competencies.
• Performs other duties as assigned.
Education Requirement
High school diploma or equivalent and Vocational/technical training in relevant area
Experience Requirement
0-2 years of experience in a healthcare, clinical setting, or a relevant area
An equivalent combination of education and experience may be considered.
All experience must be paid and in the same related field.
Part-time and PRN experience will be prorated based on hours worked per week.
Volunteer work and internships for academic credit are not counted.
