Job Description:
Job Summary
Responsible for performing patient care under supervision and collecting and analyzing patient data. Formulates individualized care plans and determines progress toward outcomes. Coordinates care delivery and promotes a safe practice environment. Advocates mutual respect with patients, families, employees, and healthcare providers.
Essential Functions
• Assesses patients and conducts screenings using standard scales, documenting findings as required.
• Prioritizes and documents patient problems in collaboration with the patient, family, and healthcare team.
• Documents changes in patient condition and evaluates the effectiveness and timeliness of care and interventions.
• Utilizes safe medication practices, including the five rights of medication administration, barcode scanning, and knowledge of medication actions, dosages, indications, and side effects.
• Provides teaching and educational materials to patients and families, assessing and documenting their understanding.
• Delegates tasks appropriately to ancillary personnel and performs all clinical nursing skills according to policy and procedure.
• Delivers age-specific and culturally sensitive nursing care tailored to the patient population.
• Transcribes physician orders accurately and manages patient admissions, transfers, and discharges.
• Performs nursing care for patients undergoing elective or emergency procedures.
• Performs other duties as assigned.
