• Conduct comprehensive psychosocial assessments through in-depth interviews, observations, and meticulous reviews of medical records to evaluate emotional, social, and environmental factors that may affect patient recovery.
• Provide empathetic counseling and support for patients and their families, helping them navigate adjustment issues and emotional challenges that arise throughout the rehabilitation process.
• Work closely with our interdisciplinary rehabilitation team to develop and implement individualized treatment plans that address each patient’s unique psychosocial needs.
• Assist patients in identifying and accessing essential community resources, including support groups, vocational rehabilitation services, and housing assistance, to facilitate a smoother transition during and after rehabilitation.
• Educate patients and their families about available community resources and help prepare them for post-rehabilitation integration into the community.
• Play an active role in discharge planning, ensuring that patients have a clear, supportive path towards home or other care settings.

Our patient population includes individuals from various backgrounds dealing with a range of conditions, such as strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. The role of the LCSW is critical in helping these individuals regain not only their physical capabilities but also emotional resilience and independence. • A Master’s degree in Social Work (MSW) from an accredited institution.
• Current and active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.
• Strong understanding of psychosocial dynamics, adjustment issues, grief, and effective coping mechanisms.
• Exceptional communication and interpersonal skills, capable of fostering strong therapeutic relationships with patients and family members.

• Previous experience in a rehabilitation or medical setting, with a focus on patient-centered care.
• Familiarity with telehealth platforms and digital communication tools to provide virtual support.
