Key Responsibilities
  • Conduct thorough, age, and culturally appropriate biopsychosocial assessments that encompass mental health, substance abuse, and medical needs within the primary care framework.
  • Facilitate individual, couples, and family counseling sessions, applying evidence-based techniques for assessment, diagnosis, and treatment planning.
  • Implement crisis intervention strategies and make appropriate referrals to community resources and medication assistance programs to ensure patients receive needed services.
  • Document all client interactions meticulously, including mental health evaluations, treatment plans, progress notes, and suicide risk assessments, in compliance with organizational policies.
  • Work collaboratively with Quality Improvement teams to enhance service delivery processes, collecting and analyzing data to inform quality care initiatives.
