Make a Difference

As a Post-Acute Care Coordinator LPN, you will provide collaborative, planning, referral and monitoring of treatment and support services to adults with severe mental illness, persons with chronic addictions, or children with severe emotional disturbances and their families. The Post-Acute Care Coordinator LPN works closely with the Remote Patient Monitor to coordinate collaborative patient care, case management and/or coordination experience in a setting that includes a collaborative, iterative process of assessing, diagnosing, planning to achieve mutually agreed upon patient goals, implementing interventions, and evaluating progress toward goals.

Exceptional Skills and Qualifications

  • Graduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience required.
  • Licensed as an LPN by the Indiana Professional Licensing Agency required.
  • Two years of nursing experience preferred but not required.
  • Current Indiana Driver\'s License with valid vehicle insurance
