Job description

Bridges Healthcare, Inc. has an immediate opening for a long-term temporary Kinship Navigator.

$27.00 per-hour/25-35 hours per-week, Monday through Friday.

This is a non-benefited position, and the duration could be up to 1 year.

PURPOSE and POSITION OVERVIEW

Maintain children in kinship and fictive kin care by providing collateral and case management services.

DUTIES and RESPONSIBILITIES

  • Conducts initial and comprehensive assessment of children and their kinship caregivers to identify service needs.
  • Provide in home-based support services to children and families.
  • Demonstrate a comprehensive understanding of available services and resources for kinship caregivers and provide support in accessing those services.
  • Maintain client records.
  • Develop and maintain collaborative relationships with other service providers, including participation in monthly meetings with DCF's Regional Gatekeeping Liaison.
  • Provide transportation support where needed.
  • Be available for on-call duties.
  • Perform any other department or agency-related duties or special projects as directed by supervisor.

QUALIFICATIONS and COMPETENCIES

Lived experience as either kinship caregivers or have lived in a household that provided kinship care required

Bachelor's degree preferred.

Ability to work flexible schedule.

Knowledge of community integration strategies and techniques.

Good communication and documentation skills.

Computer literacy.

Ability to prioritize and organize schedules/demands.

CPR/First Aid certified or willing to become certified.

Valid driver's license and access to reliable transportation required.
