Take your RN career beyond the bedside. Join Guthrie Home Health in Waverly as an RN Case Manager and receive up to a $25,000 sign-on bonus while providing meaningful, one-on-one care to patients in their homes.

This is a field-based home health role for an RN who enjoys independence, patient education, care coordination, and building trusted relationships with patients and families. You will manage a home health caseload, coordinate services with a multidisciplinary team, and help patients recover, heal, and remain independent in the place they are most comfortable: home. Typical hours are 8:00 AM โ€“ 4:30 PM, Monday through Friday, with on-call and weekend rotation, approximately one weekend per month. You will have the flexibility to plan your day around your patient caseload while still having the support of an experienced Home Health team. This may be a strong fit for an RN with experience in home health, visiting nurse services, hospice, Med/Surg, acute care, rehab, skilled nursing, or another patient-centered clinical setting. Prior home health or community health experience is preferred, but RNs with strong assessment skills, sound clinical judgment, and the ability to work independently are encouraged to apply.

Position Summary:

Responsible for the case management and provision of quality home nursing services to patients within the agency's philosophy, policy, and Standards of Community Health Nursing Practice.

Education, License & Cert:

Graduation from an approved School of Nursing, Bachelor's Degree in Nursing preferred.

Experience:

Oneโ€year previous nursing experience with Medical/Surgical Nursing required. Prior Public Health/Community Health experience preferred.

Essential Functions:

  1. Plans, implements, manages, and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient's needs are met and quality care provided in accordance with Federal, State, and program guidelines.
  2. Is responsible for ensuring that he or she can meet the yearly competency requirements. Attends staff meetings and educational opportunities.
  3. Must provide nursing care in the patient's environment.
  4. Manages the activities of the multiโ€disciplinary team providing care to patient.
  5. Demonstrates effective time management, flexibility and priority setting in patient care.
  6. Must have excellent verbal and nonverbal communication skills and facilitate communication between team members and patients.
  7. Independent judgement and excellent planning and problemโ€solving skills are required.
  8. Is responsible for LPN supervision.
  9. Must have reliable transportation.
  10. Must be able to accept on call assignments.

The payrate for this position is $83,200 - $112,569.60 per hour.
