Care Management Coordinator - Remote
Location: 100% Remote (Candidates must reside in the Hidalgo, TX area)
Job Type: Full-Time
About the Role
We're seeking a customer-focused Care Management Coordinator to support administrative care management activities in a fast-paced, remote call center environment. In this role, you'll serve as a primary point of contact for members, scheduling assessments, coordinating services, documenting member information, and delivering outstanding customer support. Bilingual candidates are highly encouraged to apply.
What You'll Do
  • Make outbound calls and answer inbound calls to schedule member assessments and services
  • Serve as the primary scheduling contact for members, providers, and internal care teams
  • Resolve scheduling issues and provide exceptional customer service
  • Maintain accurate member documentation in compliance with state and regulatory guidelines
  • Communicate with members, caregivers, and legal representatives regarding appointments and services
  • Manage multiple systems while documenting interactions and updating member records
  • Meet daily productivity goals and departmental turnaround times
  • Maintain professionalism, attendance, and quality standards in a high-volume call environment
Required Qualifications
  • High school diploma or GED
  • 1-2 years of customer service, administrative support, healthcare support, or related experience
  • Experience handling a high volume of inbound and outbound phone calls
  • Strong computer skills, including Microsoft Outlook, Excel, and Microsoft Office
  • Excellent typing and data entry skills
  • Strong communication, organizational, and multitasking abilities
  • Ability to work independently in a fast-paced remote environment while meeting performance goals
  • Must reside in the Hidalgo, Texas area
Preferred Qualifications
  • Bilingual (English/Spanish)
  • Call center experience
  • Home health, managed care, healthcare scheduling, or medical office experience
  • Experience working in environments with strict deadlines and productivity metrics
  • Familiarity with healthcare documentation and member service processes

  • Handle approximately 30 member interactions per day
  • Meet scheduling and outreach turnaround time requirements
  • Deliver exceptional customer service while maintaining productivity and quality standards
  • Accurately document member information and follow regulatory guidelines
  • Effectively manage multiple systems while assisting members
