Job description

Responsibilities

The Day Center CNA (Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care.

  • Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting.
  • Document care provided in accordance with established policies, guidelines, and applicable regulations.
  • Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews.
  • Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed.
  • Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner.
  • Collaborate with multidisciplinary teams in care planning for participants.
  • Monitor appetite and look for changes in functional ability and skin condition.
  • Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary.
  • Report any variance, accident, or unusual occurrence to the appropriate personnel.
  • Assist with loading and unloading of participants from vehicles.
  • Assist the transportation department with participants’ take-home supplies.
  • Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities.
  • Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed.
  • Attend in-service and continuing education programs as scheduled and necessary.
  • Other duties as assigned.

REQUIRED

  • Active and valid state-issued Certified Nursing Assistant (CNA) license
  • Current CPR certification or the ability to obtain CPR certification. Acceptable vendors for certifications are from either American Heart Association and/or American Red Cross.
  • High School Diploma or GED equivalent

PREFERRED

  • Experience in a care setting for frail and elderly patients participating in a group environment
  • Bi-lingual
