Support Care Worker - Hospital Overflow Program
Position Type: Full-time, Time-Limited Contract ending December 31, 2026
Schedule: Friday-Tuesday, 11pm-7am
Reporting to: Program Manager, Hospital Overflow Program Compensation: $24.00


This position is part of the CUPE [Local # 2289-05] bargaining unit and is subject to the terms and
conditions outlined in the collective agreement.


Since 1933, Haven Toronto has been a drop-in centre where elderly homeless, marginally housed
and socially isolated men can feel safe, where they can form and cultivate friendships, and
become part of a community. Our vision is a safe and just community in which elder homeless
men can live a quality and dignified life.


Summary:

As part of the support staff team, the Support Care Worker will support clients in the
overnight program at Haven Toronto, a program of St. Michael's Homes, delivered in partnership
with St. Michael's Hospital, Michael Garron Hospital, and the East Toronto Health Partners OHT.
Individuals presenting at local emergency departments who do not require immediate medical
attention are redirected to Haven Toronto for safe, supervised overnight rest.


The Support Care Worker provides client-centered support to individuals experiencing
homelessness, substance use, mental health challenges, and complex social needs.
Working as part of a multidisciplinary team, the Support Care Worker is responsible for greeting
clients, handling referrals, registering participants for programs, scheduling appointments,
coordinating referrals, and offering general support to help clients access services effectively.


Key Responsibilities
Welcome and orient new clients, including registration and enrolment processes.
Conduct general assessments of clients' physical health, mental health, and overall well-being;
provide direct support and/or refer to appropriate community resources as needed.
Document client needs, observations, and interactions, and consult with colleagues to support
coordinated care.
Assist clients in scheduling and attending appointments, including follow-up with relevant
professionals and service providers.
Develop and maintain supportive, respectful relationships with clients, monitoring well-being
and encouraging positive engagement and connections.
Support access to on-site services, including showers, laundry, meals, clothing, and
recreational spaces.
Receive, record, and distribute client mail, phone messages, and essential supplies (e.g.,
clothing, towels, food, recreational items, harm reduction supplies) as appropriate.
Maintain a safe, respectful, and welcoming environment; monitor client interactions and
intervene appropriately when required.
Provide crisis intervention in accordance with Haven Toronto policies and procedures,
including documenting actions taken and follow-up recommendations.
Administer first aid or contact emergency services as required.
Complete accurate documentation, including daily activities, incident reports, and program
statistics.
Assist with receiving and organizing food, donations, and supplies; maintain inventory and
equipment as required.
Work collaboratively with colleagues, volunteers, and clients to support a positive and
inclusive environment.
Ensure the facility is clean, organized, and safe, including maintaining common areas and
addressing hazards.
Perform opening and closing duties
Complete and document hourly safety and security checks.
Stock supplies throughout the building as needed.
Support the coordination and direction of volunteers, as requested by management.
Represent Haven Toronto in a professional, respectful, and constructive manner at all times.


Qualifications:
Post-secondary diploma in Social Services, Addictions, Recreation, or Mental Health.
Demonstrated awareness of issues related to aging, homelessness, addictions, mental
health, harm reduction, and food and income insecurity.
Demonstrated skills at working respectfully and constructively dealing with people of
diverse backgrounds.
Experience working within a trauma-informed and anti-oppression framework
Strong interpersonal, communication, and documentation skills
Ability to manage competing priorities in a fast-paced environment
Demonstrated experience in creatively engaging older economically marginalized people.
Knowledge of downtown Toronto social resources is an asset.
Valid First Aid and CPR qualifications.
