House of Hope, a well‑established nonprofit providing residential treatment for individuals with substance use disorders, is seeking a Medical Care Coordinator. This role is ideal for someone who is highly organized, compassionate, and committed to supporting clients through their recovery journey.
As a Medical Care Coordinator, you will play a crucial role in ensuring individuals receive necessary medical services, attend appointments, and stay consistent with prescribed medications.
Key Responsibilities

  • Assist clients in accessing ongoing services, including medical, dental, vision, housing, financial assistance, and legal advocacy.
  • Support clients in obtaining or reactivating disability benefits, Medicaid, and other needed resources.
  • Ensure timely medication distribution and support medication compliance.
  • Conduct daily medication checks and document compliance in the EMR system.
  • Monitor new or updated medications to ensure appropriate documentation and continuity.
  • Complete and upload all incidental funds paperwork with accuracy and adherence to agency policies.
  • Assist counselors with discharge planning.
  • Perform additional duties as assigned.

Required Qualifications

  • High School Diploma required; bachelor’s degree preferred.
  • 1 year of Care Coordinator experience.
  • 2 years of Behavioral Health experience.
  • Experience working with individuals with substance use disorders preferred.
  • Strong communication and problem‑solving skills.
  • Ability to work effectively with a diverse population.
  • Computer literacy and comfort with EMR systems.
