Job description

The case management role is a critical link in the chain between providers, medical services and social services that may be received by the person served. Purposed to stabilize the individual in all domains of life including securing housing, increasing access to food, aiding in workforce readiness and job searching, linking and coordinating medical, psychiatric and all other support services.

Case Managers consult with identified professionals, family members and others include their input into the Person Centered Planning process, inform all involved stakeholder, and monitor the status of the recipient in relationship to the treatment goals. The case manager assumes the roles of advocate, broker, coordinator, and monitor of the service delivery system on behalf of the recipient. The Professional provides coordination of movement across levels of care, directly to the person and their family, and coordinates discharge planning and community re-entry following hospitalization, residential services and other levels of care.

With effective case management individuals will experience decreased crises episodes, and increased community tenure, time working, in school or with social contacts, and personal satisfaction and independence. Through supports based on the individuals’ needs, consumers will reside in independent or semi-independent living arrangements, and be engaged in the recovery process.
